Client Services
The AIDS Council's Client Services Department works to enhance the stability, longevity, and quality of life for persons living with HIV and AIDS by offering a wide range of programs and support services for both clients and their families. Services include, but are not limited to:
Case Management
If you are living with HIV or AIDS, Case Management can help connect you with the services you need to make and your life stable and healthy. It does not matter if you have just been diagnosed or have been living with HIV/AIDS for a long time.
There are different types of services a Case Manager can provide depending on your individual needs. A Case Manager will discuss services you may need and how to get them, including referrals to benefits, assistance and programs. Case Management is a partnership between you and a trained HIV Case Manager. It can be short-term or long-term depending on your need.
- Help you find medical care including a HIV specialist
- Assistance in scheduling and keeping medical appointments
- Accompany you to medical appointments if you need assistance and help talking to the doctor
- Provide referrals to community services and resources
Housing Case Management
Supportive Housing Case Manager works together with the client to obtain and/or maintain housing Case Manager assists the client with:
- Housing stabilization
- Development of independent living skills
- Budget counseling
- Landlord negotiation
- Locating and maintaining available housing and financial resources
- Referral to community resources
- Assistance and follow-up services provided for 1 year
Health Insurance Access Program (HIAP)
If you are uninsured or underinsured the Health Insurance Access Program will assist you:
- With completing ADAP, ADAP Plus, Home Care, and APIC
- By advocating on you behalf with providers
- By providing follow-up to ensure successful enrollment
- With transitioning to comprehensive health care, which includes prescription and medical coverage
Mental Health Services
- Referrals and assistance with enrollment into mental health counseling services
- Follow-up and monitoring to ensure adherence
- Mental Health Support Groups offered at various locations
- Limited individual mental health counseling
- Issues covered but not limited to: HIV disclosure, bereavement, and treatment adherence
Substance Abuse Services
- Individual and group counseling and support
- Referrals, assistance, and support for inpatient or outpatient drug/ alcohol treatment programs
- Harm reduction counseling (work with you to plan for reducing risk of HIV infection)
- Assistance with accessing clean needles
Emergency Assistance
Assistance is provided on an emergency basis to AIDS Council clients and clients recieving case management with an approved HIV community provider.
- Emergency rent and utility assistance
- Minor renovations
- Moving assistance
- Weatherization kits (AIDS Council clients only)
- Food pantry, toiletries, and food vouchers (AIDS Council clients only)
Transportation Assistance
- Assistance locating and enrolling in transportation program
- Coordinating non-emergency transportation for medical appointments, substance abuse treatment, and mental health counseling
- Providing limited transportation for non-medical appointments including support groups, legal appointments, and social events
Client Social Events
Socialization is important to a client's well-being. The AIDS Council provides opportunities for clients to socialize in an informal and relaxed setting. Some of our events include:
- Client summer cook-out
- Holiday dinner
- Lunch and dinner program (Hudson)
Holiday Sponsor Program
The Holiday Sponsor Program matches volunteer sponsors with clients during the holidays. Based on client needs, volunteer sponsors provide gifts to clients and their family.
