No agency is better suited to deal with the complexities of the HIV/AIDS crisis than the AIDS Council. Our case managers work with individuals and their families to build skills to manage their lives. Our goals are to empower our clients, improve the quality of their lives, enhance health outcomes, prevent them from passing the virus to others, and keep families together.
The AIDS Council's team of professionals help people living with HIV/AIDS access critical services such as:

The AIDS Council of Northeastern New York’s Prevention Services Department is dedicated to stopping the spread of HIV in Northeastern New York. Our targeted prevention initiatives help people most at risk change behavior in order to reduce the rate of HIV, sexually transmitted diseases, and hepatitis. Our cutting edge methods include:
HIV / Hepatitis C Counseling and Testing & STD Screening
Confidential counseling and testing in accessible community settings. Sessions include individualized risk assessment and risk reduction planning. Counseling and testing services are also co-located in substance abuse treatment facilities and other human service agencies.
Multiple Session Skill-Building Workshops
Through this series of interactive workshops, participants can learn to make effective changes and healthier life choices. Programs offer basic HIV, hepatitis, and STD information within a real-life context to build valuable life skills that impact areas of life beyond HIV/STD prevention. Project HOPE - HIV/STD Prevention Resource for the GLBT Community
Project HOPE offers a continuum of services with flexible hours including nights and weekends to meet community needs. Click here for more information on Project HOPE services.
Community Re-Entry
Helps individuals who are HIV negative or of unknown status to make healthy transitions into the community from jail, prison, and residential treatment programs. To request services or make a referral, click here.
Transitional Case Management (Recovery Readiness/Relapse Prevention)
Case managers work with active drug users who are HIV negative or of unknown status to prepare for referrals into treatment and to encourage acceptance of treatment. We also assist individuals with prior treatment history. To request services or make a referral, contact our Transitional Case Managers.
Enhanced Outreach
An updated version of street outreach in targeted neighborhoods of high need, enhanced outreach uses multiple encounters to develop relationships that set the stage to link community members to services.
Safety Counts
Group and individual activities help participants to develop personal risk reduction goals, enhance social networks, encourage and support the recovery process, and prevent relapse. Peer Associates Initiative
Trusted community members provide outreach, information, and support through Safety Counts/Enhanced Outreach and Project HOPE. We invite you to contact Prevention Services to find out more information on becoming a Peer Associate.
Professional Training
A variety of HIV and related trainings provided to educators, substance abuse treatment providers, and other human service and public health professionals. Trainings are provided at our training sites or at your site. CASAC/CPP/CPS credit hours apply to most trainings. Group and individual interventions with HIV infected individuals and/or their partners to reduce transmission and improve health outcomes. To request services or make a referral, please contact us.
Criminal Justice Initiative
A variety of prevention and support services within designated state correctional facilities. Services include anonymous HIV counseling and testing, sexual health education, peer education training, and transitional planning for HIV+ inmates.
